Creating an order form

How to add pre-screening questions to your event.


Order forms give you the ability to add an additional set of questions to ask guests to fill out prior to completing registration.

To setup an order form:

  1. Under the “Order Form” settings, turn on “Customize order form”
  2. Input what you’d like to ask your guest in the “Prompt” input
  3. Select which type of “Answer Format” you want for the prompt
    1. Text: A simple text-input for guests to type anything they want
    2. Select Multiple: Allow guests to choose multiple options from a list you provide
    3. Select One: Allow guests to choose one option from a list you provide
  4. If you’d like to require an answer, toggle on “Required” for each question
